What is a RACI matrix?

A RACI matrix clarifies participation in project activities by identifying who is Responsible, Accountable, Consulted and Informed. It exposes gaps, duplicated ownership and unclear approval routes before they affect delivery.

How to use the RACI Matrix

  1. Step 1List the project's key activities or decisions as rows.
  2. Step 2Add roles or stakeholders as columns and assign the appropriate RACI code.
  3. Step 3Review warnings, resolve missing accountability and export the agreed matrix.

Formula / How it works

Each activity should normally have exactly one Accountable role and at least one Responsible role.

Example

Assign a Project Manager as Accountable and a Design Lead as Responsible for design approval.

What do the letters RACI mean?

Responsible performs the work, Accountable owns the outcome, Consulted provides two-way input and Informed receives updates.

Can an activity have more than one Accountable person?

It is usually best to assign exactly one Accountable role so the final decision and ownership are clear.

Can one person be both Responsible and Accountable?

Yes, especially in a small team, provided the assignment reflects the real working and approval responsibilities.
